    CEO Baru Wellcome Trust Berkunjung ke OUCRU di Indonesia dan Vietnam: Perkuat Kemitraan Riset Kesehatan yang Berusia Beberapa Dekade

    Pada 22-23 Januari 2024, Dr. John-Arne Røttingen berkunjung ke Oxford University Clinical Research Unit (OUCRU) di Indonesia dan Vietnam, didampingi oleh Dr. Paul Schreier selaku COO dan CEO interim. Delegasi ini menerima sambutan hangat dari para dewan direktur, termasuk Direktur OUCRU, Prof. Guy Thwaites; Direktur OUCRU Indonesia, Prof. Kevin Baird; dan para pimpinan rumah sakit dan organisasi mitra.

    Selama kunjungan ini, Dr. Røttingen mengadakan beberapa pertemuan produktif dengan para pimpinan rumah sakit dan institusi tuan rumah OUCRU di Indonesia dan Vietnam. Di Indonesia, pertemuan dilakukan dengan Prof. Ari Fahrial Syam, Dekan Fakultas Kedokteran Universitas Indonesia (FK UI) – institusi yang menjadi tuan rumah OUCRU Indonesia – serta Prof. Badriul Hegar Syarif, Direktur Indonesia Medical Education and Research Institute (IMERI). Pertemuan ini memberikan pihak Wellcome kesempatan untuk memahami lebih dalam aktivitas dan pencapaian OUCRU beserta mitra-mitranya di Indonesia sekaligus membuka peluang untuk kolaborasi lebih lanjut.

    Di Vietnam, delegasi Wellcome Trust bertemu dengan Dewan Direksi Rumah Sakit Penyakit Tropis Kota Ho Chi Minh yang juga merupakan institusi tuan rumah OUCRU Ho Chi Minh City. Diskusi tersebut dipimpin oleh Direktur Rumah Sakit, Prof. Nguyen Thanh Dung. Dr. Røttingen menghargai kemitraan erat antara OUCRU dan mitra lokalnya dan mengakui kontribusi signifikan yang telah OUCRU berikan dalam memajukan penelitian kesehatan dan menerjemahkan temuan tersebut ke dalam praktik-praktik klinis.

    Dr. Røttingen dan delegasi Wellcome Trust bertemu dengan pimpinan Rumah Sakit Penyakit Tropis Kota Ho Chi Minh, Vietnam.

    Delegasi Wellcome juga mengikuti tur fasilitas laboratorium OUCRU yang dilengkapi dengan peralatan penelitian mutakhir, banyak di antaranya dibiayai oleh Wellcome Trust. Laboratorium-laboratorium ini memiliki peran yang sangat krusial bagi OUCRU dalam mempelajari dan mengatasi berbagai penyakit yang umum ditemui di wilayah operasinya. Dr. Røttingen juga memperoleh penjelasan tentang pedoman keamanan dan protokol ketat di laboratorium, menegaskan komitmen OUCRU terhadap keselamatan para peneliti dan lingkungan sekitarnya.

    Kunjungan ditutup dengan sesi interaktif antara Dr. Røttingen dan staf-staf OUCRU. Dalam pidatonya, ia menjelaskan misi Wellcome untuk memperkuat penelitian dan inovasi kesehatan secara global, dengan fokus khusus pada kesehatan mental, penyakit menular, dan penjalinan kerja sama dengan pemberi dana lainnya.

    Sebagai bagian dari Program Asia dan Afrika Wellcome, OUCRU telah mendapatkan dukungan yang substansial dari yayasan tersebut sejak OUCRU berdiri pada tahun 1991. Kemitraan ini memainkan peran penting dalam penelitian berdampak di Asia Tenggara dan secara konsisten telah berkontribusi pada pencapaian visi Wellcome untuk memajukan penelitian kesehatan secara global.

    Wellcome Trust’s New CEO Visits OUCRU in Indonesia and Vietnam: Strengthening a Decades-long Partnership in Health Research 

    On January 22-23, 2024, Wellcome’s new CEO, Dr John-Arne Røttingen visited the Oxford University Clinical Research Unit (OUCRU) in Indonesia and Vietnam, accompanied by Dr Paul Schreier, the COO and outgoing acting CEO. The delegation received a warm welcome from the director board, including OUCRU Director Prof. Guy Thwaites, OUCRU Indonesia Director Prof. Kevin Baird, and the leaders of partnered hospitals and organisations.  

    During the visit, Dr. Røttingen had productive meetings with leaders of OUCRU’s host hospitals and institutions in both Indonesia and Vietnam. In Indonesia, discussions were held with Prof. Ari Fahrial Syam, the Dean of the Faculty of Medicine Universitas Indonesia (FMUI) – the institute hosting OUCRU Indonesia, and Prof. Badriul Hegar Syarif, the Director of Indonesia Medical Education and Research Institute (IMERI). The meeting provided insights into the works of OUCRU and partners in Indonesia, opening up possibilities for further collaborations.

    In Vietnam, the Wellcome Trust delegation met with the Director Board of the Hospital of Tropical Diseases in Ho Chi Minh City, the host institution for OUCRU Ho Chi Minh City. The discussions were led by Hospital Director, Prof. Nguyen Thanh Dung. Dr. Røttingen values the close partnership between OUCRU and its local host partners, recognising the significant contributions they make in advancing health research and translating those findings into clinical practice.

    The delegation also toured OUCRU’s laboratory facilities, which house state-of-the-art research equipment, including many funded by the Wellcome Trust. These well-equipped labs are critical in OUCRU’s efforts to study and address prevalent diseases in the region. Dr. Røttingen was also briefed on the lab’s strict safety guidelines and protocols, underscoring OUCRU’s commitment to the safety of researchers and the surrounding environment.

    The visit concluded with an engaging session between Dr Røttingen and OUCRU staff. He delivered a speech outlining Wellcome’s mission to enhance global health research and innovation, with a focus on mental health, infectious diseases, and collaboration with other funders.  

    OUCRU, part of the Wellcome Asia and Africa Programme, has received substantial support from the UK-based foundation since its inception in 1991. This partnership plays a significant role in OUCRU’s impactful research in Southeast Asia and contributes to the realisation of Wellcome’s vision for promoting health research worldwide.

    Cuộc thi Luận văn Ba Phút (3MT)

    Cuộc thi Luận văn Ba Phút (3MT) được tổ chức thường niên bởi Đơn vị Nghiên cứu Y học lâm sàng bệnh nhiệt đới Madihol Oxford (MORU) và Đơn vị Nghiên cứu Lâm sàng Đại học Oxford (OUCRU) nhằm nâng cao kỹ năng học thuật, thuyết trình và truyền thông khoa học của Nghiên cứu sinh Tiến sỹ.

    Các thí sinh sẽ trình bày nghiên cứu khoa học của mình trong một bài thuyết trình ba phút. Thời gian ngắn đòi hỏi họ phải tóm tắt nội dung một cách ngắn gọn, dễ hiểu cho khán giả không chuyên.

    Cuộc thi có ba vòng loại, và các thí sinh xuất sắc nhất sẽ tham gia vòng Chung kết. Năm nay, sự kiện Chung kết diễn ra tại Thành phố Hồ Chí Minh, Việt Nam. Hội đồng giám khảo gồm các nhà khoa học và học sinh trung học sẽ tham gia bầu chọn người chiến thắng.

    Hội nghị Sinh viên MORU-OUCRU (MOSC)

    Bên cạnh Cuộc thi 3MT, Hội nghị Sinh viên MORU-OUCRU (MOSC) cũng được tổ chức hàng năm. Hội nghị là cơ hội giúp các Nghiên cứu sinh từ MORU và OUCRU gặp gỡ, kết nối, và chuẩn bị cho chặng đường nghiên cứu tiến sĩ cũng như bảo vệ luận văn của mình.

    Năm 2023 là lần đầu tiên chúng tôi tổ chức chuyến đi ngoại khóa kéo dài 2 ngày cho các thành viên. Chuyến đi giúp các thành viên mở rộng mạng lưới kết nối và phát triển hơn nữa trong lĩnh vực học thuật.

    2023 Three-Minute Thesis and MORU-OUCRU Student Conference

    Three-Minute Thesis (3MT)

    The MORU-OUCRU Three-Minute Thesis (3MT) is an annual competition organised by the Mahidol Oxford Tropical Medicine Research Unit (MORU) and Oxford University Clinical Research Unit (OUCRU) to enhance the academic, presentation and research communication skills of PhD students. 

    Participants are challenged to explain their research in a concise three-minute presentation, tailored for a non-specialist audience. 

    The competition consists of three qualifying rounds, and the shortlisted participants advance to the Grand Final 3MT Competition. This year, the Grand Final took place in Ho Chi Minh City, Vietnam and was attended by a live audience. A panel of expert judges and invited local high school students vote to determine the winners, who receive exciting prizes.

    Alongside the 3MT Competition, the MORU-OUCRU Student Conference (MOSC) is held annually. This conference provides an excellent platform for students from MORU and OUCRU to meet, connect, share their stories and effectively prepare for their PhD studies and vivas. 

    Students embrace the joy of learning and connection at the 2023 MORU-OUCRU Student Conference.

    In 2023, we introduced an exciting addition to the conference, an enriching 2-day away trip for the competitors and students. This trip offered unique opportunities for networking and further academic growth.

    Nhóm Nghiên cứu Lao OUCRU Báo cáo tại “Há»™i nghị Khoa học Há»™i Hô hấp Việt Nam – Há»™i Phổi Pháp Việt 2023”

    Há»™i Hô hấp Việt Nam phối hợp vá»›i Há»™i Phổi Pháp Việt tổ chức Há»™i nghị Khoa học Há»™i Hô hấp Việt Nam – Há»™i Phổi Pháp Việt 2023 (VNRS – AFVP 2023) tại thành phố Hạ Long, Việt Nam từ ngày 3 đến 5 tháng 11 năm 2023.

    Nhóm Nghiên cứu Lao tại OUCRU do TS BS. Lê Hồng Vân dẫn đầu đã trình bày ba bài báo tại há»™i nghị: “Giá trị của GeneXpert MTB/RIF ultra và đánh giá lại giá trị của Adenosine Deaminase trong chẩn Ä‘oán lao màng phổi” (BS. Lê Thị Thu Thảo), ” Giá trị của xét nghiệm 3gene RNA signature trong chuẩn Ä‘oán lao phổi” (BS. Đặng Thị Thanh Thảo) và “Nhiá»…m lao tiềm ẩn ở nhân viên y tế công tác tại các cÆ¡ sở khám và Ä‘iều trị lao: tỉ lệ và yếu tố nguy cÆ¡” (ThS BS. Nguyá»…n Hoàng Văn).

    Trong số 30 bài trình bày được thực hiện tại Phiên báo cáo Nghiên cứu của hội nghị, nhóm OUCRU đã nhận giải nhất cho bài báo cáo của BS. Đặng Thị Thanh Thảo và giải nhì cho ThS BS. Nguyễn Hoàng Văn.

    Các bài báo cáo này đã cho thấy những tiến bá»™ má»›i nhất trong nghiên cứu khoa học và tạo Ä‘iều kiện cho các chuyên gia y tế áp dụng các phÆ°Æ¡ng pháp chẩn Ä‘oán đổi má»›i vào việc chăm sóc bệnh nhân, nâng cao Ä‘á»™ chính xác chẩn Ä‘oán và cải thiện kết quả Ä‘iều trị cho bệnh lao phổi,  cÅ©ng nhÆ° nhấn mạnh tầm quan trọng của việc sàng lọc nhiá»…m lao tiềm ẩn cho nhân viên y tế làm việc tại các cÆ¡ sở khám và Ä‘iều trị lao.

    Nhóm nghiên cứu Lao cũng đã công bố bài báo “Chẩn đoán mới cho bệnh lao: từ nghiên cứu đến lâm sàng” trên Tạp chí Y học lâm sàng (ISSN: 1859-3593) của TS BS. Lê Hồng Vân, ThS BS. Nguyễn Thanh Vân và PGS TS. Nguyễn Thụy Thương Thương. Toàn văn bài báo được truy cập theo đường dẫn.

    OUCRU Tuberculosis Group Reported at the “Congress of Vietnam respiratory society – French-Vietnamese Association of Pulmonology 2023”

    The Vietnam Respiratory Society in collaboration with the French-Vietnamese Association of Pulmonology organized the Congress of Vietnam respiratory society – French-Vietnamese Association of Pulmonology 2023 (VNRS – AFVP 2023) in Ha Long city, Vietnam on 3-5 November 2023.

    The Clinical Team of Tuberculosis Research Group at OUCRU leading by Dr. Le Hong Van presented three papers at the congress: “The value of GeneXpert MTB/RIF ultra and re-evaluating performance of Adenosine Deaminase for diagnosis of pleural tuberculous effusion” (Le Thi Thu Thao, MD), “The accuracy of the 3-gene RNA signature in Pulmonary Tuberculosis diagnosis” (Dang Thi Thanh Thao, MD) and “Latent tuberculosis infection among healthcare workers working at tuberculosis clinical settings: prevalence and risk factors” (Nguyen Hoang Van, MD, MSc).

    Out of the 30 presentations delivered at the Research Report Session of the congress, OUCRU team was awarded the first prize for the presentation of Dr Dang Thi Thanh Thao, and the second prize for Dr Nguyen Hoang Van.

    These presentations showcased the most recent advancements in scientific research and empowered medical professionals to incorporate innovative diagnostic methods into their patient care, enhancing diagnostic accuracy and improving patient outcomes regarding pulmonary tuberculosis as well as highlighted the importance of Latent Tuberculosis Infection screening for healthcare workers working in tuberculosis clinical settings.

    The TB group also published the article “Novel tuberculosis diagnosis: from bench to bedside” in the Journal of Clinical Medicine (ISSN: 1859-3593) by Dr Le Hong Van, Dr Nguyen Thanh Van and Dr Nguyen Thuy Thuong Thuong. Full text of the paper is available in this link.

  • Nghiên cứu má»›i: Thá»­ nghiệm dùng Dexamethasone bổ trợ trong Ä‘iều trị bệnh nhân lao màng não người lá»›n nhiá»…m HIV (Thá»­ nghiệm ACT HIV)

    • Thá»­ nghiệm ACT HIV, được thá»±c hiện bởi OUCRU, nhằm xác định xem Dexamethasone có phải là má»™t thuốc bổ trợ hiệu quả, giúp giảm tá»· lệ tá»­ vong, tàn tật và các biến chứng khác ở bệnh nhân lao màng não người lá»›n nhiá»…m HIV hay không.
    • Thá»­ nghiệm có sá»± tham gia của 520 bệnh nhân tại Indonesia và Việt Nam. Kết quả cho thấy việc bổ trợ Dexamethasone vào phác đồ chuẩn Ä‘iều trị lao màng não không làm giảm số lượng bệnh nhân tá»­ vong trong vòng 12 tháng kể từ thời Ä‘iểm tham gia cuá»™c thá»­ nghiệm.
    • Nghiên cứu này nhấn mạnh nhu cầu cấp thiết toàn cầu về việc phát hiện và Ä‘iều trị sá»›m HIV và lao màng não. Đồng thời, nó cÅ©ng cho thấy tầm quan trọng của sá»± hợp tác nghiên cứu giữa hai nÆ°á»›c Việt Nam và Indonesia trong việc tiến hành các cuá»™c thá»­ nghiệm quan trọng.

    VIỆT NAM, THÀNH PHỐ HỒ CHÍ MINH – Vào ngày 12/10/2023, kết quả của Thử nghiệm dùng Dexamethasone bổ trợ trong điều trị bệnh nhân lao màng não người lớn nhiễm HIV (Thử nghiệm ACT HIV) sẽ được công bố trên tạp chí New England Journal of Medicine.

    Bệnh lao do vi khuẩn Mycobacterium tuberculosis gây ra và là nguyên nhân gây tử vong cho khoảng 1,5 triệu người mỗi năm. Lao màng não là thể lao nguy hiểm nhất của bệnh lao, giết chết hoặc gây tàn tật hơn một nửa số bệnh nhân lao đồng nhiễm HIV, kể cả những người đã được điều trị bằng các loại thuốc điều trị lao tốt nhất trong 12 tháng. Việc bổ trợ Corticosteroid Dexamethasone vào phác đồ điều trị bệnh lao giúp giảm tỷ lệ tử vong ở bệnh nhân lao màng não không nhiễm HIV. Tuy nhiên, tác dụng của Demamethasone đối với bệnh nhân lao màng não nhiễm HIV vẫn chưa được làm rõ, dù cho đa số các hướng dẫn điều trị đều khuyến nghị sử dụng nó.

    Thử nghiệm ACT HIV nhằm xác định xem Dexamethasone có phải là một thuốc bổ trợ hiệu quả, giúp giảm tỷ lệ tử vong, tàn tật và các biến chứng khác ở bệnh nhân lao màng não người lớn nhiễm HIV hay không.

    Trong 4 năm, 520 bệnh nhân lao màng não nhiễm HIV được ngẫu nhiên chia thành hai nhóm, một nhóm nhận bổ trợ Dexamethasone trong 6-8 tuần và một nhóm nhận giả dược tương tự, song song với thuốc điều trị lao. Kết quả cho thấy việc bổ trợ Dexamethasone không làm giảm số lượng bệnh nhân tử vong trong vòng 12 tháng kể từ thời điểm tham gia thử nghiệm. Tỷ lệ tử vong của nhóm nhận Dexamethasone là 116/263 (44.1%), và của nhóm nhận giả dược là 126/257 (49.0%). Sự chênh lệch này không có ý nghĩa thống kê. Ngoài ra, cũng không tìm thấy lợi ích khác của Dexamethasone.

    Kết quả thử nghiệm này có ý nghĩa quan trọng. Nó cho thấy khác với ở bệnh nhân lao màng não không nhiễm HIV, Dexamethasone không có nhiều lợi ích đối với bệnh nhân lao màng não nhiễm HIV. Thử nghiệm cũng nhấn mạnh tỷ lệ tử vong cao bởi viêm màng não với bệnh nhân HIV, và nhu cầu cấp thiết toàn cầu trong việc phát hiện và điều trị HIV và lao màng não sớm hơn, hiệu quả hơn.

    Thử nghiệm là kết quả hợp tác giữa các nhà nghiên cứu tại Đơn vị Nghiên cứu Lâm sàng Đại học Oxford (OUCRU) tại Việt Nam và Indonesia, cùng với Bệnh viện Bệnh nhiệt đới và Bệnh viện Phạm Ngọc Thạch tại Thành phố Hồ Chí Minh, Việt Nam, Bệnh viện Trung ương Cipto Mangunkusumo và Bệnh viện Trung ương Đề án Hô hấp Quốc gia Persahabatan, thủ đô Jakarta, Indonesia.

  • New Study: Adjunctive Dexamethasone for Tuberculous Meningitis in HIV-Positive Adults

    • The study underscores the urgent global need for better detection and treatment of HIV and tuberculosis. It also shows the importance of cross-country collaboration between researchers in Vietnam and Indonesia in delivering practice-defining trials.

    VIETNAM, HO CHI MINH CITY – On 12 October 2023, the results of the ACT HIV trial – Adjunctive Dexamethasone for Tuberculous Meningitis in HIV-positive Adults – were published in the New England Journal of Medicine.

    Tuberculosis, caused by the bacterium Mycobacterium tuberculosis, kills around 1.5 million people each year. Tuberculous meningitis is the most lethal form of tuberculosis, killing or disabling more than half of HIV-positive individuals who contract it, despite treatment with the best available anti-tuberculosis drugs for 12 months. In HIV-negative people with tuberculous meningitis, deaths from the disease are reduced by the addition of the corticosteroid dexamethasone to anti-tuberculosis drug treatment. However, whether dexamethasone saves lives in HIV-positive people with tuberculous meningitis is unknown, although most treatment guidelines recommend it is given.

    The ACT HIV trial sought to determine whether the addition of dexamethasone to standard anti-tuberculosis drugs reduced death, disability, and other complications in HIV-positive adults with tuberculous meningitis.

    Over 4 years, 520 HIV-positive Vietnamese and Indonesian adults with tuberculous meningitis were randomly assigned to receive 6-8 weeks of dexamethasone or an identical placebo alongside anti-tuberculosis drugs. The trial found that the addition of dexamethasone did not reduce the number of adults who died within 12 months from recruitment to the trial. There were 116/263 (44.1%) deaths in the dexamethasone group and 126/257 (49.0%) deaths in the placebo group, a difference that was not statistically significant. No other benefits of dexamethasone were identified.

    The trial results are important because they show that dexamethasone has little benefit in HIV-positive people with tuberculous meningitis, unlike HIV-negative people. The trial also highlights the unacceptably high death rate from tuberculous meningitis in HIV-positive people, and the urgent global need to detect and treat HIV and tuberculosis earlier and more effectively.

This clinical trial provides important evidence for WHO to revise their recommendations for TBM/HIV (+) patients, which will change the practice at local hospitals.”

    The trial was a collaboration between researchers at the Oxford University Clinical Research Unit (OUCRU) in Vietnam and Indonesia, together with staff at the Hospital for Tropical Diseases and Pham Ngoc Thach Hospital for Tuberculosis and Lung Disease in Ho Chi Minh City, Vietnam, and Dr. Cipto Mangunkusumo National Reference Hospital and Persahabatan National Respiratory Referral Hospital, in Jakarta, Indonesia.
